Get Reimbursed for Health and Wellness Expenses
Before you send in a claim, you will need this info:
- Your SF MRA account number*
- The type of purchase or service(s) you got
- The date that you go the purchase or service(s)
- The amount you paid
- Proof of your payment (such as a receipt)
- Who you paid the amount to (the name of store or the provider)
*Your account number is a special ID starting with 910 that was given to you in your SF MRA Welcome Letter.
Four Ways to Submit a SF MRA Claim
You can use your SF MRA funds by sending in a claim online. Or, you can send in a claim on your smartphone through the HealthEquity/EZ Receipts app. You can also send in a claim by mail or fax.
1. Online:
Visit participant.wageworks.com/sfmra. Fill out the claim form and enter your receipt(s) online.

2. Mobile App:
Download the free HealthEquity/EZ Receipts app from the Google Play Store or Apple App Store.
- Log in to your account through the app.
- Fill out the claim form and upload images of your receipts.
- Click “Submit Claim” to send your claim.

3. By Mail:
Collect your receipts for the health care cost(s) you paid for. Fill out a claim form. Don’t forget to sign the form.

4. By FAX:
- Collect your receipts for the expense(s) you paid for.
- Fill out a claim form. Don’t forget to sign the form.
- Fax your completed claim form and receipt(s) to fax number 1(866) 599-3058.
Call HealthEquity/EZ Receipts Customer Service at 1(866) 697-6078 for help finding your account number and for help with the claims process.
It may take a few business days to first confirm your bank account when using direct deposit.
How Long Does It Take?
If your claim is approved, you’ll get your money back in 3-5 business days.
Direct Deposit
If you chose direct deposit, funds will be in your bank account in 3-5 business days.
Email Updates
If you gave your email address, you’ll get updates by email.
